Root Cause Failure Analysis

Do you experience bearings, gear reducers, and electric motors continuously failing?

As a Reliability Engineer, one of the most challenging aspects of my job was conducting Root Cause Failure Analysis (RCFA's). The investigation itself wasn’t the issue; rather, it was that the findings often revealed improper installation by mechanics or electricians, which sometimes led to tension among employees. No matter how kindly I tried convey the reasons, feelings were still hurt. This is why involving a third party to determine the root cause is often the best approach.
